Stomach rinse , also commonly called abdominal pumping or gastric irrigation , is the process of cleansing the contents of the stomach. It has been used for over 200 years as a means to remove toxins from the stomach. Such devices are commonly used in people who have ingested toxins or overdose on drugs such as alcohol. They can also be used before surgery, to clear the contents of the digestive tract before it is opened.

Regardless of toxicology, gastric lavage (or nasogastric lavage) is sometimes used to confirm the rate of bleeding from the upper gastrointestinal tract. This may play a role in the evaluation of hematemesis. It can also be used as a cooling technique for hyperthermic patients.

Technique

Gastric rash involves a tube tube (such as an Ewald tube) through the mouth or nose into the abdomen followed by sequential administration and removal of small amounts of fluid. The placement of the tube in the stomach should be confirmed either by air insufflation while listening to the stomach, by testing the pH of a small amount of aspirated abdominal contents, or x-rays. This is to ensure the tube is not in the lungs. In adults, a small amount of warm water or salt is provided and, through the act of sucking, is removed again. In children, normal saline is used, as children are at higher risk of hyponatremia if they are swallowed by water. Lavage is repeated until the liquid returns does not show further stomach contents. If the patient is unconscious or unable to protect their respiratory tract then the patient should be intubated before lavage. Indication

Gastric lavage is rarely used in the treatment of modern poisoning. Some authorities actually suggest that it is not used routinely, if ever, in a poisonous situation. Lavage should only be considered if the amount of poison swallowed is potentially life-threatening and this procedure can be done within 60 minutes after consumption. Lavage is also an early treatment for duodenal atresia in newborns, a condition in which the small intestine is closed distally to the stomach, causing food and fluid to accumulate in the stomach. When the body temperature rises above 40 ° C, rinse the stomach with salt ice can also be a measure of aggressive cooling in a medical emergency.

Contraindications

Lavage is contraindicated when patients experience impaired, unprotected airway and in patients at risk for gastrointestinal or perforation bleeding. Relative contraindications include when toxicity due to corrosive substances (T54), hydrocarbons (T53), or to toxins that have an effective antidote. it is also contraindicated in the case of aluminum phosphide poisoning.

Complications

Many complications have been reported, although it appears that serious complications are rare. The most dangerous risk is aspiration of pneumonia, which is more likely to occur if hydrocarbons are ingested in patients without protected airways. Other complications include laryngospasm, hypoxia, bradycardia, epistaxis, hyponatremia, hypochloremia, water poisoning, or mechanical injury to the stomach.
