A suprapubic cystostomy or suprapubic catheter (also known as vesicostomy or epicystostomy ) is a connection made through surgery between the bladder urine and skin are used to drain urine from the bladder in individuals with normal urinary flow obstruction. The connection is not through the abdominal cavity.
The flow of urine can be blocked by swelling of the prostate (benign prostatic hypertrophy), urethral traumatic disorders, congenital urinary tract defects, or obstructions such as kidney stones entering the urethra, and cancer. It is also a common treatment used among spinal cord injury patients who are unable or unwilling to use intermittent catheterization to empty the bladder, and can not be declared void due to sphincter detrusor dyssynergia.
Initially, a thin tube (catheter) is placed through the skin just above the pubic bone to the bladder, often with the help of ultrasound imaging. The catheter originally remained in place for a month while the tissue around it scarred and formed a channel (sinus) between the bladder and the outside of the body. After the formation of scar tissue is complete, the catheter is replaced periodically to help prevent infection

Medical use
- A failed urethral catheter,
- Long-term use (if left in long-term catheters of the urethra may result in acquired hypospadias and repeated/chronic UTI, urinary tract infection).

Society and culture
The "suprapubic cystotomy" is a specialty of fictional physician Stephen Maturin in Patrick O'Brian's twenty-first volume series on the Royal Navy during the Napoleonic era. In modern medical terminology, "cystotomy" without "s" refers to surgical incision or puncture to the bladder, such as to remove urinary tract stones or to perform tissue repair and reconstruction. "Cystostomy" is a special operation to provide drainage.
